Our organization offers the following CPT codes for remote patient monitoring services: 99453 for initial set-up and patient education, 99454 for device supply and daily recording or transmission of alerts every 30 days, 99457 for treatment management services requiring 20 or more minutes of clinical staff/physician/QHP time with interactive communication with the patient or caregiver, and 99458 for additional 20 minutes of treatment management services. To be eligible, patients must have a face-to-face consultation and advanced beneficiary consent must be obtained and documented in the EMR. Documentation requirements include time spent assessing, reviewing, interpreting data, communicating with patients and caregivers, providing guidance on the use of digital tools, and maintaining an active feedback loop. Please note that copays and deductibles may apply and that these services can be billed with CCM, but cannot be double-counted.
